Acknowledgment of Risk

I, the undersigned, acknowledge that axe throwing is a potentially dangerous activity that involves the risk of serious injury, disability, or death. These risks include, but are not limited to:

• Accidental cuts or wounds

• Flying or rebounding axes

• Slips, trips, or falls

• Contact with other participants

• Equipment malfunction or misuse

I freely assume all such risks, both known and unknown, even if arising from the negligence of others, and take full responsibility for my participation.

Release of Liability

In consideration for being allowed to participate in axe throwing at Skyline Farms, I hereby release and hold harmless Skyline Farms, its owners, employees, agents, volunteers, and affiliates from any and all liability, claims, demands, or causes of action for injury, disability, death, or loss or damage to person or property resulting from my participation in axe throwing activities.

This release extends to all claims of negligence, including negligent supervision, instruction, or maintenance of equipment or premises.

Rules & Behavior Agreement

I agree to follow all posted rules and instructions given by staff. I understand that failure to comply may result in removal from the premises without a refund.

Key safety rules include but are not limited to:

• Only throw axes in designated areas.

• Never retrieve an axe until all participants have thrown.

• Do not throw if under the influence of alcohol or drugs.

• Closed-toe shoes must be worn at all times.

Medical & Emergency

I certify that I am in good physical condition and capable of participating in axe throwing activities. I authorize Skyline Farms staff to obtain medical assistance in case of emergency if I am unable to do so.

Media Release (Optional)
